MISSING LAPTOP RETURNED TO MAYFIELD DENTAL ARTS

Mayfield Heights, OH – August 19, 2024 – On April 3, 2024, Mayfield Dental Arts LLC (“Mayfield Dental”), notified the Mayfield Heights police that a laptop was missing from its offices.

As part of the investigation, Mayfield Dental learned that the laptop may have been in the possession of a dentist who was formerly employed with Mayfield Dental. The laptop was returned at the end of April, but the laptop had been wiped before it was returned.

Upon return of the laptop, Mayfield Dental engaged a security consultant to try to determine the scope of the information that may have been on the laptop. Unfortunately, the consultant was largely unable to determine what information had been stored on the laptop.

Although the laptop was located in a dental office and occasionally used by dentists and other employees, it was not used in the day-to-day operations of the practice. Based on Mayfield Dental’s investigation, the specific personal information that may have been on the laptop may have included names, ages, date of births, email addresses, addresses, phone numbers, social security numbers, and sensitive health information. At this time, Mayfield Dental has no evidence that the information has been used for fraudulent purposes.

Nevertheless, in an abundance of caution and in accordance with its obligations under the Health Insurance Portability and Accountability Act (“HIPAA”), Mayfield Dental has notified potentially affected patients.

Mayfield Dental regrets this incident and takes the protection of its patients’ personal information seriously. Mayfield Dental has and will continue to work closely with local law enforcement authorities to resolve this matter.
